THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Residential Treatment Services of Alamance, Inc. (“RTSA”) is committed to protecting the privacy and confidentiality of your protected health information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act (“HIPAA”) and applicable federal and state laws, including 42 C.F.R. Part 2 regarding substance use disorder treatment records.
OUR RESPONSIBILITIES
RTSA is required by law to:
• Maintain the privacy and security of your protected health information.
• Provide you with this Notice of Privacy Practices.
• Follow the terms of this notice currently in effect.
• Notify you if a breach occurs that may compromise the privacy or security of your information.
HOW WE MAY USE AND DISCLOSE YOUR INFORMATION
RTSA may use or disclose your protected health information for the following purposes:
Treatment
We may use and share your information to provide, coordinate, or manage your healthcare and treatment services. This may include communication with physicians, counselors, nurses, pharmacies, hospitals, behavioral health providers, and other treatment professionals involved in your care.
Payment
We may use and disclose your information to bill and receive payment for services provided to you from Medicaid, insurance companies, managed care organizations, or other payors.
Healthcare Operations
We may use and disclose your information for operational purposes such as quality improvement activities, staff training, accreditation, licensing, compliance reviews, audits, and administrative activities necessary to operate RTSA programs.
Required by Law
We may disclose information when required by federal, state, or local law, court order, or legal process.
Public Health and Safety
We may disclose information when necessary to prevent a serious threat to health or safety or when required for public health reporting.
Substance Use Disorder Records
Information related to substance use disorder treatment is protected under federal law (42 C.F.R. Part 2). RTSA will not disclose these records without your written consent unless otherwise permitted or required by law.
YOUR RIGHTS
You have the right to:
• Request a copy of your medical records.
• Request corrections to your records.
• Request restrictions on certain uses or disclosures.
• Request confidential communications.
• Receive a list of certain disclosures of your information.
• Obtain a paper or electronic copy of this notice.
• File a complaint if you believe your privacy rights have been violated.
COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with RTSA or with the U.S. Department of Health and Human Services. Filing a complaint will not affect your care or services.
